Client Rights and Complaints

Your experience and satisfaction with services are important. If you have a concern about services you received, you have several options available. You may choose to complete one or more of the steps below:

  1. Request Your Health Care Records
    You have the right to request a copy of your health care records directly from the licensee (provider). To request records, please submit your request - Authorization to Release of Medical Records. You will have to complete a written authorization form and may be asked pay a reasonable fee for copies, consistent with applicable laws and regulations.
  2. Contact the Texas Behavioral Health Executive Council (BHEC)
    If you would like to ask questions about behavioral health licensure or regulation in Texas, you may contact the Texas Behavioral Health Executive Council:

  3. File a Consumer Complaint with the Texas Office of the Attorney General
